A consensus statement, developed by the Prehospital Stroke System of Care Consensus Conference, includes recommendations for prehospital destination plans for acute ischemic stroke (AIS) patients in rural, suburban, and urban communities. The statement comes from the American Academy of Neurology, American Heart Association/American Stroke Association, American Society of Neuroradiology, National Association of EMS Physicians, National Association of State EMS Officials, Society of NeuroInterventional Surgery, and Society of Vascular and Interventional Neurology, and is endorsed by the Neurocritical Care Society. The paper features three sets of recommendations for stroke systems of care (SSOCs) in rural, suburban, and urban communities, taking into account differences in resources, hospital certifications, geography, and population density. Additionally, it seeks to inform prehospital providers about new models of AIS care, especially thrombectomy, and how they affect the SSOCs. "Early stroke recognition, effective triage and transport, and timely and effective hospital- and provider-based stroke treatment are each a critical link in an effective SSOC," the statement authors write. "Stakeholder collaboration to form an SSOC should be driven by the singular purpose of maximizing stroke outcomes for our patients." This statement updates the most recent AHA SSOC policy statement published in 2019.
